Chronic health conditions can be costly. Our chronic and acute care program gives members support for more than 35 chronic conditions, including heart failure, asthma and cancer.
For members whose health is at risk, it’s vital to encourage healthy behaviors and apply proven clinical approaches. Our care management program is designed to help patients get back on the path to wellness and achieve a sense of well-being.
