Driving clinical value for Aetna® members

  1. Creating innovative programs to improve health outcomes. Like programs that improve how members transition out of the hospital and reduce chances of being readmitted.
  2. Using technology to deliver a better member experience. For example, being able to easily check the status of your prior authorization request on the Aetna Health℠ website or app. Or receiving alerts when your prior authorization status changes.
  3. Adding new tools and services to increase engagement and help members access appropriate services. Like scheduling time to meet with a nurse case manager to talk through your clinical needs, benefits and more.

We pay attention to your needs so you can get or stay healthier. Our clinical teams work closely together to see that care is appropriate and coordinated for you. At the same time, the Aetna network team promotes value-based solutions with our providers. Together, they help to increase the quality of the care you receive.

 

Spotlight: Aetna Clinical Collaboration program

 

After a hospital stay, you may need to follow specific instructions. This may include transitioning to a rehab facility, managing new medications or meeting with different doctors. This isn’t easy. Some people end up returning to the hospital. We want to do our part to avoid unneeded returns.

 

To help, Aetna launched the Aetna Clinical Collaboration (ACC) program. We partner with facilities to better support Medicare members when it’s time to leave the hospital. An Aetna nurse helps make sure you have the support and care you need during this important transition.

 

The program started in October 2024, and within the first six months, it helped with over 4,000 cases. We’re proud to say this has helped our members have a better experience and suffer fewer readmissions and ER visits.
