Our focus on social determinants of health

Social determinants of health are non-medical factors that can affect your health. These include:

 

  • Housing
  • Nutrition
  • Transportation
  • Financial security

 

For example, poor nutrition can contribute to health issues and unreliable transportation can prevent you from getting to your medical appointments. When you cannot access healthy foods or the care that you need, you are more likely to have poor health outcomes.

 

Health plans, doctors, community organizations and the government work together to remove these barriers for those in need. This gives everyone the chance to live their healthiest lives.

 

Our approach

 

Addressing social determinants of health is part of keeping you healthy. That’s why Aetna supports programs in some communities to help members find stable housing, access nutritious food, have reliable transportation and manage financial barriers to care. More specifically, we have:

 

  • Invested in housing
  • Supported food drives
  • Worked with school districts to improve student mental health
  • Provided free health screenings

 

As part of CVS Health, we can engage with you in more convenient ways. Local CVS Health locations conduct screening questionnaires and help us reach you – even if you don’t often see doctors.

 

We use data to proactively identify and address members’ needs. This helps us assist Medicaid members who may need extra help getting to the doctor, paying for food or securing housing.

 

A national company with local reach

 

We work closely with community organizations and state governments to address local challenges.

 

We have over 40 Community Health Councils across more than 10 states. We also work with over 450 community partners to improve health outcomes. These Community Health Councils and partnerships combine local expertise with our capabilities.

 

Our approach can be seen in Wayne County, Michigan. Research showed food insecurity, housing instability and economic challenges in the region. Aetna then worked with Catholic Charities of Southeast Michigan and Wayne Metropolitan Community Action to find solutions. Our partnership helps these organizations address social determinants of health. They provide community members with:

 

  • Housing support
  • Home repairs
  • Employment assistance
  • Food help
  • Support for finding health care
 

We also launched a special call center called Reach Engagement and Community Help (REACH). REACH works to address the social needs of our Medicaid members before they cause issues. The REACH team performs needs screenings, then connects members with the right local care and social services organizations.

We know that health is not one-size-fits-all. Community outreach and research help us create connection points to address specific needs.

 

Measurements for success

 

Tracking our results and adjusting our programs accordingly helps us support our members.

 

By using research and supporting communities on the ground, we are better able to address the root causes of health disparities, not just their symptoms. For example, Emanual County, Georgia, had high rates of diabetes and obesity. We found that they had limited food outlets. So, we helped to form the Emanuel County Community Health Council and launched the Lazarus Project Food Pantry with the Beacon of Hope Church of God.
