What is the Medication Therapy Management program?

Helping you make sure your drugs are safe and effective

Our Medication Therapy Management (MTM)* program helps you and your doctor manage your medicines. If you qualify, we’ll sign you up automatically. Read on to learn more about the cost, how to qualify and how the program works.

If you qualify, a pharmacist will review your drugs. They can then talk with you about:
 

  • How to get the most from your drugs
  • Any side effects or reactions
  • Any questions or concerns you have
  • Ways to save money on your drugs

We sign you up automatically if you qualify. But you can choose to leave the program at any time.

What part of Medicare covers MTM programs?

Medication Therapy Management is covered by Medicare Part D. If you have Part D coverage, one of our pharmacists can review your drugs. This is called a comprehensive (full) medication review. If they find a problem, they might do a targeted medication review (TMR) next to look at the issue more closely.

What does the MTM program cost?

There's no added cost to be in our MTM program.

  • You qualify for our MTM program if all three of these apply to you:

     

    1. You have three (3) or more of these conditions:

      • Respiratory disease
        • Chronic obstructive pulmonary disease (COPD)
        • Asthma
        • Chronic lung disorder
      • Bone disease
        • Osteoporosis
        • Osteoarthritis
        • Rheumatoid arthritis
      • Chronic heart failure (CHF)
      • Diabetes
      • Dyslipidemia
      • Hypertension
      • Alzheimer’s disease
      • End stage renal disease (ESRD)
      • Mental health
        • Schizophrenia
        • Depression
        • Bipolar disorder
        • Disabling mental health conditions
      • HIV/AIDS
    2. You are likely to spend more than $1,276 in 2026 on prescription drug costs (includes what you and your plan pay).

      AND

    3. You take eight (8) or more maintenance drugs that are covered by your Medicare Part D plan.


      How else you may qualify: You also may qualify if you’re an at-risk beneficiary (ARB) under a drug management program (DMP).

     

    What happens if I qualify for the MTM program?

     

    If you qualify, we'll send you a letter to confirm your enrollment. We’ll also call you.

  • Once in the program, you’ll get a full medication review. Here’s what happens:

     

    1. A health care provider checks all your drugs to see if there are any problems.
    2. If needed, they’ll work with your doctor to fix any issues.
    3. They’ll talk with you over the phone or in person at your local pharmacy. They'll go over your current drugs (both prescription and over-the-counter) and your health conditions. This will take about 30 minutes.
    4. They’ll give you a list of your drugs and a to-do list to help you manage your medicine. Share this with your doctor.

    We do a focused review at least once every three months. After each review, we might send your doctor ideas for safer or better drug options. Your doctor will decide if they want to make changes. Your prescriptions won’t change unless you and your doctor agree.

  • Drugs that are safe for you might be harmful to others. Get rid of unused and expired drugs as soon as you can. You can do this through a community safe disposal program. Some drugs are safe to throw away at home.

     

    Find a community safe drug disposal site

     

    The best way to safely get rid of your drugs is at a drug take-back site. Find a drug take-back site near you.

     

    You can also call your pharmacy or local police for a safe drug disposal site. (Call the nonemergency number.) Some pharmacies have drug drop-off boxes and mail-back programs to help you safely get rid of your unused drugs.

     

    Mail medicines to drug disposal sites

     

    The U. S. Drug Enforcement Administration lets you mail prescription drugs back to select pharmacies or authorized sites using approved packages. You’ll find details about these sites at DEATakeBack.com.

     

    Safely get rid of your drugs at home

     

    You can safely get rid of many drugs at home through your trash or by flushing them down the toilet. Find the best way to dispose of your drugs.


    If you’re throwing drugs in trash, follow these tips:

    • Take off the labels to protect your personal info.
    • Mix the drugs with yucky stuff, like dirt or used coffee grounds.
    • Put the mixture in a sealed container, like an empty margarine tub.
