Aetna® Institutional Special Needs Plan (I-SNP)

With the Aetna Institutional Special Needs Plan (I-SNP), you get your own personal care team. We work with your primary care provider (PCP) and long-term care center to make sure your care is well-planned.

Who can join an I-SNP?

An I-SNP is a type of Medicare Advantage plan. I-SNPs can help meet your needs if you live in a long-term care place, like a nursing home.


You may qualify for an Aetna I-SNP if:
 

  • You have both Medicare Part A and Part B
  • You’ve lived (or plan to live) in an approved place for care for at least 90 days

How do you enroll in an I-SNP?


With an I-SNP, you don’t have to wait for a specific time to enroll each year like you do with other kinds of Medicare plans. You can enroll in an I-SNP as soon as you qualify. Your plan will start on the first day of the month after you enroll.  

Your personal care team

With an Aetna I-SNP, you’ll have a care team to support you. They can help you with your physical, mental and emotional health. A nurse practitioner or advanced provider will work with your PCP and long-term care staff.


They will:
 

  • Visit you at least once a month or more, if needed
  • Watch your health to catch problems early
  • Check your medicines to make sure they work well together
  • Help you keep your family and your caregivers aware of your health status

Support for your all-around health care

As an Aetna I-SNP member, you’ll get all of these benefits and more. Check your plan‘s Evidence of Coverage (EOC) for more details.

  • Hearing health

    $0 copay for an in-network annual exam and fitting for a hearing aid, plus a hearing aid allowance

  • Vision care

    $0 copay for an in-network routine eye exam and an allowance for prescription contact lenses or eyeglasses

  • Dental services

    Preventive and comprehensive dental care

  • Podiatry visits

    $0 copay for in-network routine foot care visits

  • Social needs companion

    $0 copay for one-on-one support* from a credentialed (specially trained) companion.

  • Over-the-counter (OTC) health products

    A quarterly allowance to help pay for certain health and wellness products.
