Who can join an I-SNP?
An I-SNP is a type of Medicare Advantage plan. I-SNPs can help meet your needs if you live in a long-term care place, like a nursing home.
You may qualify for an Aetna I-SNP if:
- You have both Medicare Part A and Part B
- You’ve lived (or plan to live) in an approved place for care for at least 90 days
How do you enroll in an I-SNP?
With an I-SNP, you don’t have to wait for a specific time to enroll each year like you do with other kinds of Medicare plans. You can enroll in an I-SNP as soon as you qualify. Your plan will start on the first day of the month after you enroll.
Your personal care team
With an Aetna I-SNP, you’ll have a care team to support you. They can help you with your physical, mental and emotional health. A nurse practitioner or advanced provider will work with your PCP and long-term care staff.
They will:
- Visit you at least once a month or more, if needed
- Watch your health to catch problems early
- Check your medicines to make sure they work well together
- Help you keep your family and your caregivers aware of your health status
