With the ${company}® Institutional Special Needs Plan (I-SNP) you’ll have a personal care team in your corner to help with your overall health. We work with your primary care provider (PCP) and long-term care facility to provide seamless coordinated care.

Who is eligible for an I-SNP plan?

An I-SNP is a type of Medicare Advantage plan. I-SNPs can help meet the medical needs of people living in a long-term care setting. For instance, a skilled nursing facility. 

 

You may be eligible for membership in our plan if:
 

  • You have both Medicare Part A and Part B
  • You’ve lived (or plan to live) in a participating facility for at least 90 days or longer

How do you enroll in an I-SNP plan?


Unlike other plans that have a Medicare annual enrollment period, I-SNP enrollment is open at all times. Whenever you’re eligible, you can enroll in a plan. It will start on the first day of the month following your enrollment.

With an Aetna I-SNP, you’ll have a personal care team ready to support you physically, mentally and emotionally. You’ll get all-around support from a nurse practitioner or advanced practice provider who will work with your primary care provider and nursing home team.

 

They’ll help ensure you get quality care and will:

 

  • Visit you at least once a month or more, if needed
  • Monitor your condition(s) to spot any problems early on
  • Review your prescriptions to ensure they work well together
  • Be available to you, your family, caregivers and care team to share your health status

Hearing health

$0 copay for annual exam and hearing aid fitting, plus an allowance for a hearing aid.

Vision care

$0 copay for routine eye exam and an allowance for prescription contact lenses or eyeglasses.

Dental services

Preventive and comprehensive dental services.

Podiatry visits

$0 copay for routine foot care visits.

Non-emergency transportation

$0 copay for non-emergency trips to plan approved locations.

Over-the-counter (OTC) health products

A quarterly allowance to help pay for certain health and wellness products.
