With our health maintenance organization (HMO) plans, you have access to a network of doctors and hospitals. These plans require you to use network providers for your medical care. Also, you may need a referral from your primary care provider (PCP) for hospital care and specialist visits. HMO plans offer limited out-of-network benefits.

What is Medicare Advantage (Part C)?
A Medicare Advantage (Part C) plan combines Original Medicare Part A (hospital insurance) and Part B (medical insurance) in one plan. It usually includes prescription drug coverage and may offer additional benefits such as dental, vision and fitness. There is often no additional premium charge.
Medicare can be complicated, so here’s a simple guide to understanding Medicare Parts A, B, C and D.
Medicare Parts A and B, also known as Original Medicare, are provided by the government. Part A is your hospital care. It covers things like inpatient care, home health and hospice care.
Part B is your medical care. It covers things like doctors visits, outpatient care and home health care, medical equipment, and more. Part B usually requires a premium that can often be deducted from your Social Security checks.
Part C plans, also known as Medicare Advantage plans, are run by private insurance companies, like Aetna® Medicare. Medicare Advantage plans are a lot like Original Medicare. But they can include valuable extra benefits that Original Medicare doesn’t. These benefits can include dental, hearing and vision care, and fitness memberships.
And finally, there’s Part D. Part D plans offer prescription drug coverage. You can get Part D on its own or by choosing a Medicare Advantage plan that includes Part D. A Medicare Advantage plan that includes Part C and Part D coverage can often give you extra savings. When you enroll into a Part D or Medicare Advantage plan, you may have an additional monthly premium.
To get a Medicare Advantage plan, you simply need to qualify for Original Medicare. To see these qualifications, visit medicare.gov.
Before enrolling in a specific plan, ensure your health care providers, pharmacies and hospitals are in that plans network. We recommend you verify this before receiving care.

Getting back on your feet starts with the right support
After repeated falls, Carl needed more than quick fixes. He needed a path forward. With help from an Aetna Care Manager, he and his wife got the right care to get back on his feet and moving again.
Watch the story to see how it all came together.
Cheryl: At the very beginning of last year, well, Carl started having some back pain. And then, he fell on the deck. And then two days later he fell again and then a third time, and that was the point at which we rushed him to the hospital.
Carl: I had spent my whole life as an athlete. I played baseball professionally. I'm playing with my grandchildren, and now, now I couldn’t even stand up. It was devastating.
Cheryl: After Carl's first hospital stay, I received a call from an Aetna care manager. Her name was Julie. And you know, she was so warm, friendly and compassionate.
Julie: Carl was hurting. He was upset that he couldn't play with his grandkids, and this really bothered him.
Julie: I wanted to help get him back up and playing ball.
Cheryl: After months of appointments and treatment, his back was still bothering him, and I told Julie how important it was that Carl feel strong enough and healthy enough to play with his grandchildren. And she said, “How come Carl isn't in rehab?”
Julie: When I spoke with Carl, I helped him understand that rehab is a place where you get exercise and physical therapy more intensively than if you were in the home setting or if you went to outpatient therapy.
Carl: What Julie said to me really hit home.
Cheryl: Within 24 hours Carl was at the rehab of our choice.
Carl: And now, I feel as strong as I've ever felt in my life. I am so grateful for that conversation with Julie.
