  • Extra Benefits Card

    Members with a qualifying chronic condition get a monthly Extra Supports allowance on an Extra Benefits Card to help pay for certain everyday expenses including healthy foods, over-the-counter (OTC) products, personal care products, transportation and utilities. Members without a qualifying chronic condition can use the monthly allowance to purchase OTC products only.

     

    Disclaimer: If you use this program for rent or utilities, Housing and Urban Development (HUD) requires it to be reported as income if you seek assistance. Contact your local HUD office if you have questions.

  • Dental and vision coverage

    You get an annual benefit amount (allowance) for certain dental services not covered by Medicaid at an in-network dental provider. For vision, you  get an annual allowance for prescription glasses or contacts, plus a $0 copay for an in-network annual routine eye exam.

  • Care team

    A support team to help manage your care and get you the health and community services you may need. They will review your personal needs, coordinate your benefits, and build a customized care plan.

  • Prescription drugs

    Do you take medicine regularly? You won’t have to pay any copays on tier 1 and tier 2 prescription drugs we cover at in-network pharmacies.

You can do a lot with your health plan when you log in or create an account on your Member Portal. Search for providers, manage plan benefits, find your member ID card and more. Plus, you can even log in to your Member Portal on our mobile app.
