Members with a qualifying chronic condition who meet all other eligibility requirements can use the monthly allowance to help pay for additional everyday expenses, including healthy groceries, personal care products, transportation, and utilities.

For D-SNP members with a qualifying chronic condition, the plan provides a monthly allowance to help pay for certain everyday expenses, including: healthy foods, over-the-counter products, personal care products, transportation, and utilities. 

 

Members without a qualifying chronic condition can only use the monthly allowance to purchase over-the-counter products. 

 

That’s on top of our medical coverage, the dedicated support of a personal care team and $0 copays for covered Tier 1 prescription drugs at in-network pharmacies. Plus, our plans include dental, vision and hearing benefits. 

 

Not eligible for a D-SNP? Aetna has other Medicare Advantage plans with added benefits for those with limited income and resources. If you're eligible for the federal Extra Help program, Aetna has select Medicare Advantage plans that provide additional benefits.

Joan: I’ve always been clumsy.


Ginny: Don’t say it like that, mom.


Joan: Well, it happens a lot. I even fell walking my grandkids home from school. And my time with those adorable little ones is everything to me.

Joan: It was so easy. And so refreshing to be cared for like that.


Ginny: Katie helped mom go from “I’m just not going to deal with that” to “Now I can get to my appointment.” What a relief!


Katie: My goal was to help Joan feel more confident getting around. And that we’re looking out for her.


Ginny: Katie also helped mom get a fall detection device. If anything happens, it sends an alert to me.


Joan: Now Ginny worries about me a lot less. And I don’t worry as much about getting around anymore.

D-SNPs are low- to no-cost health insurance plans.

 

D-SNP members get both Medicare and Medicaid benefits. On top of that, D-SNPs offer more coverage and benefits than Original Medicare. D-SNP members also have access to a personal care team. Your care team can help you overcome common roadblocks to well-being.

D-SNP members get the benefits of a Medicare Advantage plan while keeping Medicaid. With both plans, you could get more complete medical coverage.

 

Every D-SNP is different, so be sure to check coverage in your area. For instance, Aetna offers D-SNPs that include benefits beyond medical coverage, such as:

 

  • A dedicated care team to help you understand your benefits, coordinate provider visits, manage your medications and more
  • Dental, vision and hearing coverage
  • Prescription drug coverage, including $0 copays on covered Tier 1 prescription drugs at in-network pharmacies
  • An Extra Benefits Card with a monthly benefit amount (allowance) to help  pay for certain everyday expenses.

People who are dual eligible meet the requirements for both Medicare and Medicaid. You must be dual eligible to qualify for a D-SNP.

You can check before you enroll. A licensed agent can help you look up your provider. Many plans also have provider directories on their websites. Aetna has a free online tool that lets you search for in-network providers and D-SNPs in your area.

Aetna D-SNP networks include care providers dedicated to your health and well-being. Plus, the added benefits and services that come with an Aetna D-SNP make it easier to get the care and support you need to feel your best.

No, you won’t lose your Medicaid benefits. In fact, you must be enrolled in a Medicaid program to enroll in a D-SNP. So access to your Medicaid benefits will continue.

D-SNPs are low- to no-cost health plans with a goal of helping you save money while getting quality care. For instance, Aetna D-SNPs include dental, vision and hearing coverage, plus prescription drug coverage, at no added cost. Plus, Aetna D-SNPs include a monthly benefit amount (allowance) on an Extra Benefits Card to help you stay on budget.

  • Initial Enrollment Period (IEP): This is when you first become eligible for Medicare.  It starts three months before your 65th birthday and continues for seven months.
  • Annual Enrollment Period (AEP): This occurs annually from October 15 to December 7. During this time, you can pick a new D-SNP (or any Medicare Advantage plan), switch from Original Medicare to Medicare Advantage or change your Part D coverage.
  • Open Enrollment Period (OEP): This occurs annually from January 1 to March 31. It allows beneficiaries who are already enrolled in a Medicare Advantage plan to make certain changes to their coverage.
  • Special Enrollment Period (SEP): There may be other times when you are able to enroll in a D-SNP. This can include if you move, or if there is a disaster declared by FEMA. An SEP is also available three months after you begin Medicaid coverage. And an SEP is available for full benefit dual eligible individuals (FBDE) aligning Medicaid coverage and Medicare D-SNP so they’re provided by the same insurance company.  To check your eligibility for an SEP, call a licensed Aetna agent at 1-833-771-3451 ${tty}, 8 AM to 8 PM, Monday to Friday.
