On July 17, 2025, Outcomes determined that one of the accessed items included individuals’ names and one or more of the following: demographic information, medical provider names, health insurance information, and medication information. Note that Social Security numbers were not involved.

What we are doing: To help prevent a similar incident, Outcomes has implemented and enhanced safeguards and security measures to further protect their email system, and they have implemented enhanced employee training regarding phishing emails.
