Chronic Condition Special Needs Plan (C-SNP)
Do you qualify for an Aetna® C-SNP?
You may be eligible to enroll in an Aetna C-SNP if:
- You have a Medicare plan
- You live in one of 25 participating states in one of our covered counties
- You have at least one of the eligible chronic conditions*

Ready to enroll?
To get started, you’ll need the contact information for your primary care provider (PCP) or specialist. This should be the provider who treats you or has treated you for your chronic condition. They'll need to confirm (attest to) that condition. We’ll need this to fill out the Prequalification Assessment Tool (PQAT)(PDF).
Next, we’ll send a Verification of Chronic Condition (VCC) form to the provider who will confirm (attest to) your chronic condition. They’ll let us know if you have an eligible chronic condition and qualify for a C‑SNP. We need this form sent back to us before the end of the second month that you’re enrolled in the plan.
