How to enroll in Medicare plans

When you can enroll

How do you know if it’s time to enroll? It may be your time if it's a few months before or after your 65th birthday. Or it may be your time to choose next year's plan.

Do you qualify?

 

Looking for a Medicare Advantage plan (Part C)? To qualify, you’ll need to be enrolled in Original Medicare first (Medicare Parts A and B).

Ready to enroll?

 

When you’re ready, there are three ways you can enroll:

Online

You can compare and enroll in Part C and D plans on our website. Get details on what each plan covers. And see which Aetna Medicare plan may work best for your needs.

Phone

You can enroll in an Aetna Medicare plan over the phone. Call to speak with a licensed agent at ${dynamicPhone} ${tty}, ${hours}.

Mail

You can ask us to send you a paper enrollment kit. Call a licensed agent at ${dynamicPhone} ${tty}, ${hours}, to have one sent to you. Then, follow the steps in the form to complete it and send it to us.

Dual Eligible Special Needs Plan (D-SNP)

 


Do you qualify?

 

You may be eligible to enroll in an Aetna® D-SNP if:

 

  • You already have Medicare and Medicaid
  • You live in one of 30 participating states in one of our covered counties

 

You can use the plan locator on this page. Check if your state and county are listed. If they’re not, then we don’t have a D-SNP in your location at this time.

Ready to enroll?

 

If you qualify for an Aetna® D-SNP, there are three ways you can enroll:

Online

You can review and enroll in plans on our website. Get details on what each plan covers. And find a D-SNP that’s right for you.

Phone

You can talk to a licensed agent to see if you qualify and to enroll. Call us at ${dynamicPhone} ${tty}, ${hours}.

Mail

You can call us and ask us to send you a paper form in English or Spanish. For a paper form in another language, choose your state and county on the plan locator. Next, you’ll be able to choose a plan and download an enrollment form for that plan in other available languages. Then, follow the steps in the form to complete it and send it to us.

If your state and county aren’t listed, then we don’t have a D-SNP in your location at this time.

 

Chronic Condition Special Needs Plan (C-SNP)



Do you qualify?

 

You may be eligible to enroll in an Aetna® C-SNP if:

 

  • You have a Medicare plan

  • You live in one of 18 participating states in one of our covered counties

Ready to enroll?

 

To get started, you’ll need the contact information for your primary care provider (PCP) or specialist. This should be the provider who treats you or has treated you for your chronic condition. They'll need to confirm (attest to) that condition. We’ll need this to fill out the Prequalification Assessment Tool (PQAT)

 

Next, we’ll send a Verification of Chronic Condition (VCC) form to the provider who will confirm (attest to) your chronic condition. They’ll let us know if you have an eligible chronic condition and qualify for a C‑SNP. We need this form sent back to us before the end of the second month that you’re enrolled in the plan.

You may be eligible for an Aetna® I-SNP if:

 

  • You are a nursing home resident 

  • You live in Connecticut, New York, Ohio or Pennsylvania
