Disease management: Cost-effective care for chronic conditions
Chronic conditions can take a heavy toll on people’s health, as well as on a company’s finances. Our disease management program gives people support to understand and stick with treatment and stay productive. We provide:
- A total health perspective – We see people as people, not conditions. This holistic approach helps us deliver a helpful combination of information and support.
- Advanced technology – Our systems can help identify at-risk members, determine an appropriate level of support, find opportunities to improve care and improve overall patient safety.
- Personal attention – Eligible members have access to registered nurses to answer questions about conditions and treatment.
Aetna In Touch CareSM
Aetna In Touch Care covers all types of health conditions. The program supports members at the most risk who have the greatest opportunity for health impacts. We use:
- A nurse point of contact – Members have one-on-one nurse support via phone and email.
- Virtual support – We offer custom online programs to support members.
Improving health and safety
MedQuery®, powered by CareEngine® technology, works behind the scenes to help anticipate potential care issues and send out alerts to providers and members. It scans and compiles information from a number of touch points including:
- Medical, pharmacy, lab and behavioral health claims
- Feedback from members working with registered nurses in our health programs
- Health information in a member’s Aetna Personal Health Record*

The CareEngine is a proprietary technology platform developed by ActiveHealth Management, an Aetna company. In conjunction with clinicians, the CareEngine continuously analyzes claims and other data against evidenced-based best practices and alerts the members and their physician about possible care gaps and other inconsistencies. Information is not a substitute for diagnosis or treatment by a physician or other health care professional. The Aetna Personal Health Record should not be used as the sole source of information about the member's medical history.
