Your care manager is an advocate for your health. They start by listening to you to create a personalized care plan for your health goals and concerns. And they can continue to work with you based on your needs and coverage.

 

Your care manager can:

 

  • Help you find a doctor and schedule appointments
  • Offer education on diet and exercise
  • Share tips to lower health care costs
  • Connect you to programs and resources that may be part of your plan

 

How to access a care manager:

  • For members with certain health conditions, a care manager may reach out to you directly.
  • You can also call the number on your ID card or log in to your member website and choose “Help” to get phone, email or chat options.
