The state of Connecticut has issued new provider contract termination requirements, effective 7/1/2018.
Notice of leaving network
A health carrier and participating provider must provide at least 90 days’ written notice to each other before the health carrier removes a participating provider from the network or the participating provider leaves the network.
60-day continuation (hospitals only)
If the contract is not renewed or is terminated by either the health carrier or the hospital, the health carrier and the hospital will continue to abide by the terms of such contract, including reimbursement terms, for a period of 60 days from the date of termination or, in the case of a nonrenewal, from the end of the contract period.
Please be sure your office staff is aware of these requirements.

Each main plan type has more than one subtype. Some subtypes have five tiers of coverage. Others have four tiers, three tiers or two tiers. This search will use the five-tier subtype. It will show you whether a drug is covered or not covered, but the tier information may not be the same as it is for your specific plan. Do you want to continue?
