This CPB has been revised to state that (i) plasmapheresis, plasma exchange, or therapeutic apheresis is considered medically necessary for babesiosis if member has high-grade parasitemia (greater than or equal to 10 %), severe anemia (hemoglobin less than or equal to 10 g/dL), or hepatic, pulmonary, or renal compromise, and (ii) plasmapheresis, plasma exchange, or therapeutic apheresis is considered experimental and investigational for obsessive-compulsive disorder, and for reducing peri-operative blood transfusion.
