Alcohol Disease Management Program 
Thank you for your interest in our program.

It's a two-track program

Track One:
We find people who have concerns about their alcohol use and want our help. We do this in a number of ways, including looking at claims data. Then we contact people so they can begin treatment with their doctors.

Track Two:
Our program supports people with the care and services they need to improve their health. We help them manage their alcohol use and guide their recovery. Those in the program work with their doctors.

We also support those who have a problem with alcohol and have another health condition. These can include depression and cirrhosis.

Terms & Conditions/Privacy
The following Terms and Conditions shall govern your use of the information, tools and other content (the "Content") accessible via the Texting Pilot. You acknowledge that you have read, understand and agree to the following.

You understand that you must provide, at your own expense, all internet, telephone, mobile device, mobile network or other equipment and services necessary to access and use the Texting Pilot.

Confirmation of Age
You are 18 years of age or older or the age of majority, as specified by state law, if applicable.

TEXTING PILOT IS NOT THE PROVISION OF MEDICAL SERVICES OR ADVICE
NEITHER THE TEXTING PILOT, NOR ITS CONTENT CONSTITUTE MEDICAL, HEALTH OR PSYCHIATRIC ADVICE. THE TRANSMISSION AND RECEIPT OF CONTENT, OR COMMUNICATION VIA TEXT MESSAGING, THE INTERNET, E-MAIL OR OTHER MEANS DOES NOT CONSTITUTE OR CREATE A DOCTOR-PATIENT, THERAPIST-PATIENT OR OTHER HEALTH CARE PROFESSIONAL RELATIONSHIP BETWEEN YOU AND AETNA. THE CONTENT IS NOT INTENDED TO BE AND SHOULD NOT BE USED IN PLACE OF THE ADVICE OF YOUR PHYSICIAN OR OTHER MEDICAL PROFESSIONAL. Personal Information/Privacy
Any non-public personal information that you may provide via the Texting Pilot will be used solely for the purpose stated. We will not collect personal information about you unless you provide that information voluntarily. Aetna will not sell, license, transmit or disclose this information outside of Aetna and its affiliated companies unless (a) expressly authorized by you, (b) necessary to enable Aetna contractors or agents to perform certain functions for us, or (c) required by law.

No Modification of Benefit Plan
The Content viewed and/or received via the Texting Pilot does not replace or modify any benefit plan or materials provided in connection with any plan.

Changes in Terms & Conditions and Content
Aetna reserves the right, in its sole discretion, to change, modify, add or remove any portion of these Terms and Conditions in whole or in part, at any time. Changes in these Terms and Conditions will be effective when posted to this website. Your continued use of and participation in the Texting Pilot after any changes to the Terms and Conditions have been posted will be considered acceptance of those changes. Aetna may terminate, change, suspend or discontinue any aspect of the Texting Pilot, including the availability of any features or Content of the Texting Pilot, at any time. Aetna may also impose limits on certain features, services and Content or restrict your access to parts or all of the Texting Pilot or this website without notice or liability.

Access or Delivery to Mobile Network is not Guaranteed
Content access or delivery to a mobile device may fail due to circumstances or conditions beyond Aetna's control. Aetna may not attempt to retry delivery to your mobile device. You understand and acknowledge that network services, including but not limited to mobile network services, are outside of Aetna's control and influence and that Aetna may not be able to assist in resolution of such problems. You agree that Aetna is not responsible for your use of the service outside of the Aetna servers and is not responsible or liable for problems arising in the network outside of the Aetna servers.  Furthermore, you understand that your use of and participation in the Texting Pilot may be limited by your mobile network operator or by the capabilities or capacities of your terminal or mobile device.

Message and Data Rates May Apply 
Aetna does not charge for the Texting Pilot, but standard message and data rates may apply as provided in your mobile network plan (contact your carrier for pricing plans and details).

Opting Out
You may opt out of the Texting Pilot at any time by texting STOP, STOP ALL, QUIT, END, CANCEL, or UNSUBSCRIBE, to our short code from your mobile phone. We will unsubscribe you from our text messaging service as soon as possible thereafter.

Help
You may obtain help on the Texting Pilot at any time by texting HELP to our short code from your mobile phone.

Applicability
These Terms and Conditions shall apply to any and all of your use of and participation in the Texting Pilot. These Terms and Conditions shall survive any termination of your access to, use of or participation in the Texting Pilot. Aetna may terminate your access to or participation in the Texting Pilot if you violate any of these Terms and Conditions.

Miscellaneous
These Terms and Conditions constitute the entire agreement between Aetna and you with respect to the Texting Pilot. These Terms and Conditions shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania. You agree that any legal actions or proceedings between Aetna and you for any purpose concerning these Terms & Conditions or any obligations of you or Aetna hereunder shall be brought exclusively in a court of competent jurisdiction in Montgomery County, Pennsylvania. Any cause of action or claim you may have with respect to the Texting Pilot must be commenced within one (1) year after the cause of action or claim arises. Aetna's failure to insist upon or enforce strict performance of any provision of these Terms & Conditions shall not be construed as a waiver of any provision or right. Neither the course of conduct between you and Aetna nor trade practices shall act to modify any provision of the Terms and Conditions.

Identification
You have properly identified yourself by providing your own name and personal identifiers when enrolling in the Text Pilot and not those of any other person.
