Aetna Medical Members please Note:  Everyone enrolled in Aetna Medical on or after 4/1/08 will receive new ID cards in the mid-March time frame.  When you receive your new Aetna ID cards, please discard any previous cards.

Remember to complete an enrollment form today! 

• If you are not yet an Aetna member, find out what Aetna can do for you!  To Enroll in the Aetna HMO program, simply obtain an enrollment form from your local HR Manager or from a Benefits Fair.

 Complete your form with all necessary enrollment information, including but not limited to:

• Name, SSN, Date of Birth, plan selection, Primary Care Physician information, and dependent information.
• Simply hand your enrollment form to your local HR department for processing and you're done!  You will receive your ID card within two weeks from the time your enrollment information has been provided to Aetna.
• Already an Aetna Medical member?  No action needed!  You will remain in the Aetna program with the 4/1/08 Enrollment.

Special note on dental

Important Note to SHBP Employees:  There are no changes to the SHBP Dental Programs for 4/1/08.  Please note that the enrollment period during January and February 2008 is for the MEDICAL PLANS ONLY.  If you are currently enrolled in a SHBP Dental Program through Aetna or another carrier, you must wait for Open Enrollment in the Fall of 2008 to make changes for 1/1/09 unless you have a life-changing event (birth of child, marriage, divorce, etc).  See your Dental Member Handbook for more information on when you can make enrollment changes.
