If you are interested in joining Aetna, remember to complete an enrollment form today! 

• If you are not yet an Aetna member, find out what Aetna can do for you!  To Enroll in the Aetna HMO program, simply obtain an enrollment form from your local HR Manager or from a Benefits Fair.

Complete your form with all necessary enrollment information, including but not limited to:

• Name, SSN, Date of Birth, plan selection, Primary Care Physician information, and dependent information.
• Simply hand your enrollment form to your local HR department BY THE END OF OCTOBER for processing and you're done!  You will receive your ID card within two weeks from the time your enrollment information has been provided to Aetna.
• Already an Aetna Medical member?  No action needed!  You will remain in the Aetna program with the 1/1/08 Enrollment.
