Visit Aetna Navigator® to check out these special programs and tools. Use hmomed1 as the user name and password if you are not yet an Aetna member. After you log on, look under Benefits and then Health Programs.

With the new Personal Health Record, you can conveniently store your health information in one, confidential place. Here's what you can do:

With the Aetna Health ConnectionsSM program there's support for you every step of the way. You can get support for 34 common medical conditions, such as congestive heart failure, asthma, diabetes, hypertension, breast cancer, and migraines, to name a few. If you or a family member qualifies for the program, you will receive:

  • Educational materials and online resources
  • Nurse case management for members who have more complex needs
  • State-of-the art technology to help enhance ongoing quality of care and patient safety

Getting started in the Aetna Health Connections program is easy. Your doctor may identify you or a family member for the program, or our patient management staff may refer you. You can even self refer into the program through Aetna Navigator or by calling member services.

When you call our Informed Health® Line at 1-800 556-1555, you can:

  • Talk directly to a registered nurse. Aetna nurses can discuss more than 5000 health and wellness topics
  • Listen to audio health library.  Listen to 1000 health conditions in English and Spanish.
  • Transfer easily to a registered nurse at any time during the call.
