If you are not yet an Aetna member, find out what Aetna can do for you!  To Enroll in the Aetna HMO program, simply obtain an enrollment form from your local HR Manager or at the health fair during open enrollment.

• Simply hand your enrollment form to your local HR department for processing and you're done!  You will receive your ID card within two weeks from the time your enrollment information has been provided to Aetna.
• Already an Aetna Medical Member?  No action needed!  You will remain in the Aetna HMO program with the 7/1/08 Enrollment.
