Aetna uses its own historical claims information, adjusted to take into account health care cost inflation, in the calculation of the estimated costs. Moreover, these claim costs are limited to commercial lines of business and exclude any claims with respect to members covered by Medicare or Medicaid, or where Aetna is not the primary payer under coordination-of-benefits rules. An average cost was only considered credible if our historical claims information included an established minimum number of claims for the particular type of test or procedure. In addition, the highest and lowest claims charges for a particular type of test or procedure were considered "outliers" and were omitted from the calculation. For the user’s zip code, the average costs were displayed based on the smallest geographic area for which credible data was available. In those instances where the minimum credible claim thresholds were not met for out-of-network costs, in order to display costs for the member, we estimated the out-of-network costs using Aetna’s overall historical claims experience, and applying a factor that was derived by comparing billed amounts and payments made to in- and out-of-network providers for all PPO/Managed Choice product claims.
