How do pharmacists become participating providers in the Aetna pharmacy network?
You can request an enrollment packet in one of three ways:
- Apply online
- Fax your request to (860) 273-4876
- Mail a written request to: Aetna Pharmacy Management Provider Enrollment, RE62, 151 Farmington Ave, Hartford, CT 06156
For fax or mail requests, please include:
- Name, address and telephone number of the pharmacy
- Name of contact person at the pharmacy
- NCPDP number and NPI
Packets are sent out within a week, and the entire enrollment process usually takes about four weeks.
