Aetna also accepts paper claims that contain all the data elements of the CMS-1450 or CMS-1500 (or successor standard) forms (including but not limited to: member identification number, Social Security number, date(s) of service, complete and accurate breakdown of services). For information on submitting claims in paper form, contact the number on the back of the Member's ID card.
