Class Edit Summary
NOTE:  Drugs used for the sole purpose of weight reduction are generally not a covered benefit under most Aetna drug benefits plans (coverage may be provided under the member's Aetna medical benefits plans), and are not, therefore, included on Aetna's Formulary. 

  1. Medical Exception Criteria

  2.  The following antiobesity agents are included in this clinical policy bulletin.

    Ionamin®, Phentride®, Fastin®, Adipex-P®, Obe-Nix®, Oby-Trim®, Pro-Fast®

    diethylpropion

    Tenuate®

    benzphetamine

    Didrex®

    mazindol

    Mazanor®, Sanorex®

    sibutramine

    Meridia®

    orlistat

    Xenical®




    Drugs used for the sole purpose of weight reduction are generally not a covered benefit under most Aetna drug benefits plans. If covered as a benefit, Aetna considers Antiobesity Agents to be medically necessary for those members who meet the following criteria:

    A.  A documented:

    • Body mass index > 30 kg/m2
      OR
    • Body mass index > 27 kg/m2 with one or more of the following obesity related risk factors considered serious enough to warrant pharmacotherapy:
      a. Hypertension (systolic blood pressure > 140 mm Hg or diastolic blood pressure > 90 mm Hg on more than one occasion)
      b. Dyslipidemia
      1. LDL cholesterol > 160 mg/dL, or
      2. HDL cholesterol < 35 mg/dL, or
      3. Triglycerides > 400 mg/dL
      c. Coronary heart disease
      d. Type 2 diabetes mellitus
      e. Obstructive sleep apnea.

    AND

    B.   Member is NOT receiving two antiobesity agents at the same time.
