Policy:
- Precertification Criteria
Under some plans, including plans that use an open or closed formulary, Tazorac is subject to precertification for members greater than 35 years of age. If precertification requirements apply Aetna considers Tazorac to be medically necessary for those members who meet any of the following precertification criteria:
A. A documented diagnosis of acne vulgaris
OR
B. A documented diagnosis of plaque psoriasis
- Medical Exception Criteria
Dovonex and Taclonex are currently listed on the Aetna Formulary Exclusions List.* Therefore, Dovonex and Taclonex are excluded from coverage for members enrolled in prescription drug benefits plans that use a closed formulary, unless a medical exception is granted. Aetna considers Dovonex and Taclonex to be medically necessary for those members who meet any of the following criteria:
For Dovonex (A OR B)
A. A documented:
Contraindication to the preferred alternative Tazorac OR
Intolerance to the preferred alternative Tazorac OR
Allergy to the preferred alternative Tazorac OR
Failure of an adequate trial of one month of the preferred alternative Tazorac
Member is pregnant (Dovonex is a pregnancy Category C) OR
B. Member is documented to have been successfully treated with calcipotriene
in the past.-For Dovonex oint/cream only
For Taclonex
A. A documented:
Contraindication to the preferred alternative Tazorac or calcipotriene solution OR
Intolerance to the preferred alternative Tazorac or calcipotriene solution OR
Allergy to the preferred alternative Tazorac or calcipotriene solution OR
Failure of one month concurrent use of Tazorac or calcipotriene solution AND topical betamethasone
