An important part of life planning is to ensure that your legal affairs are taken care of - before you pass away. This ensures that your wishes are respected and helps your family to make difficult decisions during an extremely emotional time.

The Aetna Life Essentials Program takes this into consideration and offers you legal support through our Legal Reference^TM Program as administered by ARAG Services LLC^®. Features include:

• While you’re living – You can find legal forms and documents to help you plan your estate through the Legal Reference program website.
• At the end of your life – The Legal Reference program may be able to help you with in-office attorney estate planning services if you are suffering from a terminal illness.
• For your loved ones – The Legal Reference program can help your beneficiaries plan their estate.

Through the Legal Reference^TM Program, offered by ARAG Services LLC^®, you have FREE access to legal forms and information. You can create, free of charge, the following forms, including:

• Health Care Power of Attorney
• Financial Power of Attorney
• Living Will and Health Care Directive
• Basic Will Preparation

In addition, you can learn about the importance of estate planning. Plus, you will have access to information to help protect you from identity theft and a victim action kit if you need to have your identity restored.

As an Aetna Group Insurance Plan Member, you have access to the legal services through our legal reference program.

In addition:

• if you or your spouse received an accelerated death benefit payment; and
• you were either insured for supplemental life insurance or were covering your spouse for voluntary dependent life insurance when the accelerated death benefit claim is filed, you or your spouse have access to the following legal services in an in-office setting, in addition to the legal services available through our legal reference program:
  • Will Preparation Assistance
  • Health Care Power of Attorney
  • Durable Financial Power of Attorney
  • Revocable Living Trust and Tax Planning
  • Uncontested Guardianship Papers for Minor Dependent Children
  • Legal Representation for the Real Estate Sale of Your Primary Residence

This package is provided through an independent legal services company called ARAG Services LLC^® ("ARAG"). After your accelerated death benefit claim has been approved, we will automatically ask ARAG to reach out to you and your family to provide information on these services if your accelerated death benefit claim is approved.

Through Aetna Beneficiary Solutions™, your beneficiaries have access to the same legal services that you had while you were living.

If you or your spouse was eligible for in-office estate planning services but did not use those services while still living, the following legal services may be available to the executor of your estate:

• Uncontested Guardianship Papers for Minor Dependent Children
• Legal Representation for the Real Estate Sale of Your Primary Residence
• Reduced Fees (25%) for Legal Services Not Covered
• Estate Administration and Closing Service
