Medical and psychiatric case management
People with chronic medical conditions such as diabetes, asthma and heart disease are more likely to have problems with depression, anxiety, and substance abuse.
One way to help improve the health of your employees -- and to control costs -- is to offer our Med-Psych Program. It helps identify employees who have a behavioral health condition and a co-existing chronic medical condition.
Participants learn steps to manage their conditions and stick with their treatment plans. They can also receive case management services to help coordinate care.
Depression and disease management
Depression often occurs with chronic illness. That’s why we include depression screening and referrals as part of our Aetna Health ConnectionsSM disease management program. Employees can get support if they are at risk for any of more than 35 chronic health conditions.
The program helps employees deal more effectively with their doctors and take an active role in their health care. By identifying and managing a condition early, employees can help avoid complications and improve their quality of life.
Behavioral health disease management programs
Behavioral health disease management programs can give your employees extra support to help keep them more productive and healthy on the job. Programs include:
You can combine these programs with an Aetna health plan or use them with a different health plan.
Your employees will be able to use:
