Specialty pharmacy: How to enroll

Easy and fast enrollment
Ordering your prescription medications through Aetna Specialty Pharmacy is fast and easy.

• Your doctor may fax your prescription to 1-866-FAX-ASRX (1 (866) 329-2779).
• Or, you or your doctor may mail your prescription order to: Aetna Specialty Pharmacy, 503 Sunport Lane, Orlando, FL 32809. If you mail in your prescription, please send it along with a completed Patient Profile Form.  (PDF, 69 KB)
• Your doctor may also call and speak to one of our registered pharmacists at 1 (866) 782-ASRX (1 (866) 782-2779) during our normal business hours of 8 a.m. until 7 p.m. Eastern Standard Time.

Medication refills
A week prior to your refill, an Aetna Specialty Pharmacy representative will call you to schedule your delivery. The representative will confirm that you are still taking your medication, that your doctor has not changed the dose, and that you are not experiencing any unmanageable side effects.

Customized shipping & delivery
Your medication will be delivered to your home, your doctor's office, or to any other location you choose. Aetna Specialty Pharmacy makes every effort to package your medication for easy use and handling, ensuring that it is preserved until you are ready to take it. Your shipment includes necessary supplies such as syringes, adhesive bandages, Sharps containers and alcohol swabs.

What to expect with your first order
Your first order should ship within 24 to 48 hours after you speak to an Aetna Specialty Pharmacy representative. The actual ship-date will depend on whether or not we need to contact your doctor about the prescription. In your first order, you will receive a packet of important information for you to review. The following is a brief overview of items in the packet:

This letter provides an introduction to Aetna Specialty Pharmacy, and describes the state and federal regulations that apply to the forms and information sheets in the packet.

This notice describes how medical information about you may be used and disclosed as well as how you can access this information. You will need to sign a copy of this form and mail it back to us.

Form 2: Patient Information and Assignment Agreement  (PDF, 52 KB)

This form is to be signed by the patient and/or responsible party. It is required so we can file with your medical or pharmacy benefit. This form also informs you of your rights and responsibilities as an Aetna Specialty Pharmacy patient. You will receive two copies of these forms for your signature. Please sign both copies, mail one back to us and keep the other for your records.

The Refill Information Sheet contains instructions for refilling your prescriptions using the Aetna Specialty Pharmacy Automatic Refill Line. It also provides contact information for accessing clinical support 24 hours a day, 7 days a week via our toll-free number, 1 (866) 782-ASRX (1 (866) 782-2779) or TDD: 1(877) 833-ASRX (1(877) 833-2779) and the Fax 1 (866) FAX-ASRX (1 (866) 329-2779).

Form 4: Out-of-Area Emergency Preparedness Plan Notice  (PDF, 36 KB)

This is to inform you of steps to take in the event of a disaster, or other unavoidable interruption of service. Please read all of the information and keep it available for your future reference if needed.

You will also receive a medication packing slip with your order. This form is used to verify the delivery of your medications and supplies. Please call Aetna Specialty Pharmacy immediately if the packet is damaged or opened, and do not use the product. The medication packing slip will be enclosed with every delivery. Please check the package to ensure that all pertinent information applies to you (the patient). Please remember to keep us informed regarding any changes in your medication.
