How Do I Enroll?

Enrolling is easy! Once you know that you qualify for an Aetna Medicare Advantage Plan, follow the simple steps below to enroll.

1. Select the level of coverage that meets your needs.
2. Enroll in a plan using one of the following methods.
   • To enroll online: Use the Aetna Medicare Solution Center - Select the plan year, find the plan that best meets your needs, and enroll.
     
     
   • To enroll by phone: Call 1-800-529-5586 (TTY/TDD 1-800-628-3323), Monday - Friday, 8:00 a.m. - 6:00 p.m.
   
   
   
   • To enroll by mail: Download an enrollment form, then print it out.
     
     Aetna Golden Medicare (HMO) and Aetna Golden Choice (PPO) enrollment form  
     
     Aetna Medicare Open Plan (PFFS) enrollment form  
     
     Aetna Medicare Special Needs Plan (SNP) enrollment form.

Aetna Golden Medicare Plan^®
You must be entitled to Medicare Part A and continue to pay your Part B premium and Part A, if applicable. You must use network providers except for emergent care or out of area urgent care/renal dialysis.

Aetna Golden Choice™ Plan
You must be entitled to Medicare Part A and continue to pay your Part B premium and Part A, if applicable. Higher costs apply for out-of-network services. Precertification, or prior approval of coverage, is requested for certain services. Providers must be licensed and eligible to receive payment under the federal Medicare program.

Aetna Medicare Open^SM Plan
You must be entitled to Medicare Part A and continue to pay your Part B premium and Part A, if applicable. You can receive covered services from any licensed doctor or other health care practitioner that is eligible to receive payment from Medicare, agrees to treat you, and accepts the Aetna Medicare Open Plan terms and conditions. Precertification is requested for certain services.

The federal government has established periods when a person can join and change their Medicare prescription drug coverage. In general, unless you qualify for a special election period, you can change Medicare prescription drug plans once each year between November 15 and December 31.

You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for extra help, call:

• 1-800-MEDICARE (1-800-633-4227). TTY/TDD users should call 1-877-486-2048, 24 hours a day/7days a week;
• The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TDD users should call 1-800-325-0778; or
• Your State Medicaid Office.

Plans are offered by Aetna Health, Inc., Aetna Health of California Inc., Aetna Health of Illinois Inc., and/or Aetna Life Insurance Company. Coverage is provided through a Medicare Advantage organization or a Medicare Prescription Drug Plan Sponsor with a Medicare contract. Benefits, limitations, service areas and premiums are subject to change on January 1 of each year.
