Add Dental Coverage to Your Aetna Advantage Plan

With the Aetna Advantage Dental PPO Max insurance plan, you can obtain services from either a participating or non-participating dentist. You also have the flexibility to visit a dentist who does not participate in Aetna's network for covered services.

To add dental to your existing plan, complete the Dental Election\Change form (1 page) and submit the completed form to:

Dental coverage is not available in all states. You must be currently covered under an existing Aetna Advantage medical plan to add dental coverage.
