This managed care plan does not cover all of your health care expenses. Read your contract carefully to determine which health care services are covered. To contact the plan if you are a member call the number on your ID card. All others call 1-800-MY-Health. * The Individual plans are offered by Aetna Life Insurance Company directly and through an out of state blanket trust. In Pennsylvania the Individual HMO plan is offered by Aetna Health Inc. In FL, the Self-Employed can purchase a guaranteed issue group insurance plan under Small Group Reform. In CT, THIS PLAN IS ISSUED ON AN INDIVIDUAL BASIS AND IS REGULATED AS AN INDIVIDUAL HEALTH INSURANCE PLAN. |
