Effective January 1, 2011 over-the-counter medicines will not be FSA-eligible without a doctor's prescription as a result of Health Care Reform.

Ineligible Description


  The cost of an ultrasound for medical care (including prenatal care) is an eligible medical expense.

Ultrasound, Pre-Natal


The cost of an ultrasound ordered by a health care professional to monitor the growth and condition of the fetus is an eligible medical expense, even if the benefit plan does not provide reimbursement.


The cost of an ultrasound that is not ordered by a health care professional or is not Intended to monitor the growth and condition of the fetus is not an eligible expense.

Umbilical Cord Blood

See Cord Blood and Storage.
Urinalysis X The cost of urinalysis for medical conditions, including but not limited to, court ordered drug and alcohol testing both in-home and doctor's office, is an eligible medical expense.
Usual, Customary and Reasonable (UCR) Charges Above See Reasonable & Customary (R&C).
UVR Treatments X A health care professional must provide evidence of medical necessity for the cost of ultra violet light treatments to be an eligible medical expense.  Include evidence of medical necessity (i.e., a doctor's note or LOMN that specifically identifies that the recommendation and expense is for treatment of the medical condition) with the request for reimbursement.
