Supplemental Accidental Death & Personal Loss provides coverage amounts in addition to
those payable under your Basic Accidental Death & Personal Loss benefit. The amount of benefits payable under this benefit is determined by a combination of factors, including your coverage amount and the type of injury suffered.
Not all injuries caused by an accident will qualify you to receive benefit payments. The exact types of injury that qualify will be specified by your plan, but may typically include death, loss of hearing, loss of sight, or loss of limbs. To find out more about your benefits, please review the Certificate of Coverage or contact your benefits administrator.
