In most states, your beneficiary- the person you choose to receive the cash payment after your death- can call us to file a claim. Or call us to find out the status of a claim at 1-800-523-5065, Monday through Friday, 8 a.m. to 7 p.m. ET.
Your beneficiaries may also ask your employer to file the claim for them.
We will need:
A certified copy of your death certificate.
Eligibility information and signed beneficiary records from your employer.
Police or fire reports from the investigating agency if your death is caused by an accident.
We usually pay life claims within 10 to 15 business days. Claims when there is an accident can take longer.
