Pre-treatment estimate

Get a pre-treatment estimate of what your plan will cover

Need treatment and want to know what your dental benefits or dental insurance plan will cover? If you are an Aetna member and have either:

  • A pre-treatment estimate you received from your dentist while covered under Aetna Dental, or
  • A pre-treatment estimate that your prior dental plan approved

Send it with a completed Aetna claim form  (PDF, 431 KB) to:

We'll let you know if your plan covers the care your dentist suggested and send you an estimate.
Coverage under some plans may depend, in part, on whether the dentist giving the care is in our dental network.
