Heart Care for Life Program

Medical and pharmacy claims help us find members who are at risk for certain conditions. Let's say your claims show that you have a heart condition or suffered a heart attack. We may invite you to participate in the Heart Care for Life program. It helps you get the best care at the lowest out-of-pocket costs. And it helps you to stick with your prescribed drug regimen.
We'll send you a letter that explains the program. And an educational brochure (in both English and Spanish). It has tips and information on heart health and medications.

Migraine Management Program

When our claims data show that you suffer from migraines, we'll invite you to use our Migraine Management program. It can help you manage your headaches better. It may help prevent the need for brain scans, emergency room visits and hospital stays.

We'll send you and your doctor information about the program. This includes a migraine brochure and diary/action plan.
