You are entitled to request a review (appeal) of this benefit determination either by phone or in writing if you do not agree. To request an appeal by phone, you or your authorized representative should call the Member Services phone number listed on your member enrollment card. To request an appeal in writing, you or your authorized representative should submit a written request to the Aetna Life Insurance Company address listed below. More…

• Available 24 hours a day, 7 days a week.
• Instant access – no busy signals or "on hold" music.
• Personalized menu options that relate only to your plan and coverage.
• Request personal claims, eligibility and plan information.
• Perform common transactions.
• During normal business hours you can ask to talk with a customer service professional. Once you give your Aetna member ID number you will be directed to a customer service professional specially trained to help with your question.
