Add Dental Coverage to Your Aetna Advantage Plan

With the Aetna Advantage Dental PPO Max insurance plan, you can obtain services from either a participating or non-participating dentist. You also have the flexibility to visit a dentist who does not participate in Aetna's network for covered services.

To add dental to your existing plan, complete the Dental Election\Change form  (PDF, 42 KB) and submit the completed form to:

Dental coverage is not available in all states. You must be currently covered under an existing Aetna Advantage medical plan to add dental coverage.

If you've signed up for one of our HSA-Compatible plans, your next step is to open your tax-advantaged health savings account. You can use funds you deposit in this account to help pay for qualified expenses.

To open your HSA, complete the HSA Enrollment  (PDF, 82 KB), Beneficiary Designation  (PDF, 273 KB) and Custodial Agreement  (PDF, 294 KB) forms.
