The “Request for Exclusion from Paper Shut-off” form is available for you to submit your request to exclude participation in accessing explanation of benefits (EOBs) electronically. We strongly encourage providers to access EOBs online because of their convenience, the reduction of mail to their office and the continued flexibility of being able to save or print the EOB.
