Paper Claims 

For information on submitting claims in paper form, contact the number on the back of the Member's ID card.

We accept paper claims that contain all data elements contained on CMS-1450 or CMS-1500 (or successor standard) forms. Elements include: member identification number, Social Security number, dates of service, and complete and accurate breakdown of services. Other data elements may be required as well.
