The Hartford Health Disparities Collaborative (HHDC), created in 2004 as a result of a convening of over 70 local community leaders hosted by the Aetna Foundation, has focused on supporting initiatives that address racial and ethnic health disparities among Hartford residents..

Also in 2004, the City of Hartford, its three major hospitals, two federally qualified health centers, and other local partners created the Mayor's Healthy Communities Initiative (MHCI), a network of care for the city's estimated 27,000 uninsured residents using a shared database, patient ID card system, patient navigators and medical interpreters. HHDC Advisory Committee members contributed $25,000 to help MHCI launch an Education and Public Awareness campaign. Click here to see the MHCI public awareness brochure 134 KB / 1 Page. In mid-2006, MHCI began enrolling patients in the program, linking them to medical homes and providing patient navigator services. The Hispanic Health Council has assumed responsibility for day-to-day operations of MHCI, and is exploring ways to integrate MHCI's most successful elements into its core programming.
