Please see the following important notices about your RHM specific participating provider directory. This site has been designed to provide quick and easy access to participating provider directories for colleagues and their covered family members enrolled in a Trinity Health medical plan.

In order to locate providers who participate in your network(s) you must use this site rather than Aetna's public site when looking for providers in your area. On this site, you can search for Primary Care Physicians (PCP), Specialists, hospitals and other healthcare providers, such as Radiology, Dialysis, etc

Questions regarding network providers may be directed to member services by using the 'contact us' feature on Aetna NavigatorTM or by calling Aetna's Trinity Health Specific Customer Service Line at 800-544-5108.

General Access Information for All RHMs (including Saint Francis Care and JMMC)

To access a list of participating providers in your area, please select "enter doc find" at the bottom of this page.

• Once you have entered DocFind, locate your RHM from the Trinity Health RHM drop down menu.
• If searching for a Trinity Health tier 1 provider or facility please select the link under your RHM titled Tier 1 Providers and Facilities.
• If searching for an Aetna Tier 2 provider or facility please select the link under your RHM titled Tier 2 Providers and Facilities

Please note - Tier 1 providers will show in the tier 2 search as an Aetna participating provider (tier 1 status supersedes tier 2 in these cases). If looking for a tier 1 provider, please search under the tier 1 drop down.

All members: Cancer Treatment Centers of America (CTCA) - As of January 1, 2016 there is no Network or Out-Network coverage for both health care services provided by the facility; and health care services provided by physicians and other health care professionals at any of these facilities.

RHM Specific Information

Holy Cross Hospital, Ft. Lauderdale colleagues / members additional network information

Participants need to obtain an approved out-of-network referral/prior authorization for payment at the in-network benefit level before receiving services at the Acute Care Hospitals and Freestanding MRI/High Tech Radiology Centers in Broward County. A referral is required for services provided at these facilities, as well as for any physician or professional services. If you're referred to one of these providers for services and you do not obtain an approved referral/prior authorization for before receiving care, your claims will be paid at the out-of-network (tier 3) level. Also, all other out-of-network providers will be considered Tier 3 for services provided by their facilities and physicians unless it is identified that the service is not available by any other Tier 1 or Tier 2 provider.

Saint Joseph Health System, colleagues / members additional network information

All participants need to obtain an approved out-of-network referral/prior authorization for payment at the Tier 2 in-network benefit level before receiving services at Riley Hospital or Children Hospitals and Clinics or any out of network provider. A referral or prior authorization is also required for any physician or professional services. If you do not obtain an approved referral before receiving care, your claims will be paid at the Tier 3 - out-of-network level of coverage.

Mount Carmel Health System colleagues / members additional network information

New 1/1/17 - Nationwide Children's Hospital will be considered a tier 1 facility.

Your tier 1 network consists of Trinity Health facilities and those Health Partner providers who participate in clinical integration. However, if you are not able to utilize a tier 1 provider those providers who are not part of clinical integration are available at the tier 2 level of coverage.

In addition to those providers listed on DocFind as tier 1, tier 2 providers and Cornerstone Medical Services of Columbus, LLC, a Trinity Health facility, colleagues have access to Aetna's National Medical Equipment (DME) network as well as local DME Providers. Colleagues should contact Cornerstone for product availability first at 614-866-5520, and if the product is not available through Cornerstone, please use the following links to review the Aetna national and local lists:

In addition to those providers listed on DocFind as Tier 1 and Tier 2 providers, you also have access to Aetna's National DME network at the Tier 2 level of benefit. Providers are identified in the following link DME National Provider Listing.

St. Francis Healthcare Services - Wilmington, DE, colleagues / members; and

Saint Francis Medical Center - Trenton, NJ, colleagues / members; and

St. Mary Medical Center - Langhorne, PA, colleagues / members;

Please note, all covered services provided by an Aetna participating pediatric primary or pediatric specialty professional will be covered at the tier 1 level of benefit.

In addition, all covered services provided by an Aetna participating OB/GYN professional will be considered tier 1. For both pediatrics and OB-GYN this is for professional services only, not the facility services.

Facility services provided at non-Trinity Health facilities will continue to be paid at a tier 2 level of benefit. All providers will not be individually displayed in the directory for each of these specialties, however the claims will be covered at the tier 1 level of benefit.
