Holy Cross Hospital associates / members

Your network of participating providers is changing effective 1/1/08. To access a list of participating providers in your area, please select "enter doc find" at the bottom of this page.

• Once you have entered DocFind, select "Holy Cross Hospital participating providers" from the Trinity Health Ministry Organization drop down menu.

Mercy Medical Center, Mason City / New Hampton associates / members

Midland's Choice participants are required to obtain an approved referral prior to having services at the Mayo Clinic, University of Iowa Hospitals and Clinics in Iowa City. A referral is required for services provided at the hospital as well as any physician or professional services.

If a colleague or family member is referred to the University of Iowa for services, the referring physician will need to send a referral to Aetna prior to services being rendered. Referrals for emergency situations will be reviewed upon receipt at Aetna (except for weekends or off hours). Non-emergency referrals will be reviewed within 48 hours of receipt at Aetna.

For Additional information on the referral process, please contact Aetna at 800-544-5108. Again - an approved referral is required prior to obtaining services at the University of Iowa. Failure to obtain an approved referral from Aetna will result in benefits being paid at the non-network or non-PPO level.

Mount Carmel Health System associates / members

Your network of participating providers is not changing. You will continue to select your health care providers from CareNet, Mount Carmel Health Partners and Mount Carmel Behavioral Health networks of participating providers. To access a list of participating providers in your area, please select "enter doc find" at the bottom of this page.

• Once you have entered DocFind, select "Mount Carmel Health System participating providers" from the Trinity Health Ministry Organization drop down menu.

Saint Joseph Regional Medical Center associates / members

Your network of participating providers is not changing. You will continue to select your health care providers from the Select Health and Sagamore networks of participating providers. To access a list of participating providers in your area, please select "enter doc find" at the bottom of this page.

• Once you have entered DocFind, select "Saint Joseph Regional Medical Center participating providers" from the Trinity Health Ministry Organization drop down menu.

Saint Alphonsus Regional Medical Center associates / members
