Please be aware that UICH and Mayo providers may still show in the Tier 2 provider search. If you’re referred to UIHC or Mayo Clinic for services your provider needs to contact Aetna at 888-632-3862 before you receive services to obtain approval/authorization. If you do not obtain an approved referral/prior authorization before receiving care, your claims will be paid at the Tier 3 level (Out-Of-Network).
