This CPB has been revised to state that extracorporeal shock-wave therapy is considered experimental and investigational for the following: (i) hypertrophic scars of the hand caused by burn injury, (ii) myofascial pain syndrome of the trapezius, (iii) myofascial pelvic pain syndrome, and (iv) spasticity associated with cerebral palsy.
