This CPB has been revised to state that the following are considered experimental and investigational: (i) Neurogenx 4000PRO device for the treatment of Achilles tendonitis and all other indications; (ii) percutaneous electrical nerve field stimulation for the management of opioid withdrawal; (iii) Quell device (transcutaneous electrical nerve stimulation (TENS)) for the treatment of cervicalgia; (iv); Scrambler therapy/the Calmare therapy device for the treatment of Dejerine-Roussy syndrome; (v) Sprint (peripheral subcutaneous field stimulation) for the treatment of low back pain; (vi) TENS for the treatment of carpal tunnel syndrome pain; (vii) transcutaneous magnetic stimulation for the treatment of chronic pain; and (viii) combined percutaneous electrical stimulation with spinal cord stimulation for back pain and other indications..
