Key Components of the Three Component Model, 3CMTM

Aetna Depression Management is based on the Three Component Model, 3CMTM, which has been empirically validated for the treatment and management of depression. The program improves patient outcomes by:

  • Increasing treatment adherence.
  • Monitoring patient response to treatment.
  • Making treatment changes when there is a suboptimal response.

This model recognizes the crucial role that primary care physicians play in the diagnosis and treatment of depression, and it seeks to overcome common hurdles, namely the lack of time, tools and support.

1) The Primary Care Practice

The primary care practice includes both the primary care physician and the office staff. This established process and the suite of tools will help you to more easily detect depressed patients in your practice and treat them successfully, while reducing your administrative burden. Having a prepared practice means having access to:

  • Protocol guidelines
  • Diagnostic tools
    • Patient Health Questionnaire (PHQ-9)
    • Scoring key
  • Office staff training
  • Educational component with free continuing education credits

2) Care Management

Care management is carried out by a care planner. The care planner is the glue of the system, acting as a liaison among your office, your patient and mental health specialists. The care planner:

  • Monitors treatment adherence via scheduled phone contact with the patient.
  • Administers follow-up PHQ-9 to track treatment response.
  • Reports findings back to you.
  • Facilitates consultation for you with a psychiatric consultant.
  • Can refer a patient to a local mental health clinician.

3) Mental Health Specialists/Primary Care Partnership

  • A psychiatric consultant can provide input on treatment options, drug choices and drug dosages when a patient is not progressing.
  • Local mental health clinicians are available to provide psychological counseling and collaborate with you on a patient’s progress.
