Referral to a Care Planner

With Aetna Depression Management, you can refer any patient to a care planner if that patient is age 19 or older and, in your judgment, requires active treatment for depression. You do not have to base your decision on the patient’s Patient Health Questionnaire (PHQ-9) score

Always explain the role of the care planner and the care management process to any patient you refer. Let the patient know that a care planner will contact him or her in about one week. The purpose of the call is to find out if the patient started treatment. To increase the likelihood that the care planner will successfully contact the patient, please have the patient provide the best time to call. Keep in mind that the care planner serves as an extension of your ability to monitor treatment response.

To initiate care planning, complete and transmit the referral form via fax to 1-801-256-7699.


Refer Only Patients Who:

1. Require active treatment for depression (medication and/or psychological counseling) in your judgment, regardless of PHQ-9 scores.
2. Are 18 years of age or older. If a patient is at risk of suicide in a physician’s clinical judgment, the physician should follow his or her normal practice procedures for potential urgent/emergent mental health issues.
