	A nurse practitioner, nurse midwife, physician assistant or clinical nurse specialist employed by a participating physician, physician group or facility
	A nurse practitioner contracted or employed by a participating physician, physician group, nurse practitioner group who wants to be considered as a primary care provider (available for member selection as a PCP)
	A nurse practitioner, nurse practitioner group, nurse midwife or nurse midwife group who is not employed by a participating physician, physician group or facility who wants to contract directly with Aetna
	A physician or health care professional other than a nurse practitioner, midwife, physician assistant or clinical nurse specialist
