Transplant What to expect with your patients' transplant therapy Thanks to modern science, many organs can be transplanted, including: the heart, lung, kidney, liver, pancreas, bone marrow, small intestines, and others. We understand that your patient's transplant can help to increase the length and quality of their life. We want each transplant to be successful. To achieve this, we provide immunosuppressant, injectable and supportive transplant therapies. When you order transplant medications through Aetna Specialty Pharmacy, your patients benefit from our Aetna Transplant Life ManagementSM program. Through this program, your patient may receive a 90-day drug supply. We will check their benefits plan to make sure a 90-day supply is right for them. If your patient is a new transplant recipient, they will also receive a backpack starter kit. The backpack starter kit will contain the following:
If you have questions about your patients' medications, we can help. Call anytime - day or night - to talk to a registered nurse or pharmacist. This is a registered nurse who specializes in transplant therapies. Your patient's nurse will call them and confirm some key information - like their age, weight, therapy, dose, any allergies, and shipping address. The nurse may offer the patient additional education about their medication or how to use it. Your patient will also be assigned an Aetna National Medical Excellence Program nurse case manager. The nurse case manager will keep in touch with your patient for at least six months to check how they are doing. Clinical help - Your patient's nurse care manager will contact them as often as they need. If your patient has a question about their transplant or their medication, they can also talk to a nurse or pharmacist anytime, day or night. Education - We will send your patients a welcome packet to explain our services. Depending on each patient's needs, our nurses and pharmacists can offer extra education. For example, if your patient needs help with taking a self-injectable medication, their nurse care manager can walk them through the process. Medicare - Aetna Specialty Pharmacy is a Medicare provider. We maintain a Medicare team that determines if the patient's medication meets Federal guidelines for coverage. If so, this team will appropriately coordinate the patient's benefits coverage. Transplant care plan - We make sure each patient always has the right medication and dose for their needs. If your patient wants additional support, we will contact them as often as they'd like. Some people qualify for home health care. If your patient qualifies, we can arrange for a nurse to help them take their medications right in their home. And we'll keep in contact with you so you know how your patient is doing between office visits.
